A PLAN TO SMILE ABOUT

At Pierson Dental Associates, we understand the importance of quality dental health care. To combat the increasing cost of dental health care, we have developed an Dental Health Savings Plan.

Two Regular Check-ups Per Year (regularly $140)
Two Regular Cleanings Per year (regulary $170)
All necessary x-rays, including Full Mouth series or Panorex (once per 3 years) (regularly $120)
Bite Wings (once per year) (regularly $60)
15% - 20% savings on all restorative, cosmetic, endodontic, prostodontic, and surgical services.

In order to offer treatments available under the terms of this plan, we MUST adhere to the following STRICT payment policies:

The one-year membership must be paid in full at the time of enrollment.
The membership will be valid for one year from the date the annual fee is remittted.
All treatment fees are due and payable at the time service is rendered.
All fees for treatment requiring multiple visits or lab fees are due upon the first visit.
Pierson Dental Associates (PDA) and it's legal advisors, reserve the right to refuse treatment, and or terminate this membership without notice if the member's account becomes deliquent at any time for any reason. All other PDA terms and conditions of service apply.

Procedure fee discounts offered under this plan shall not apply to any other treatment already in progress prior to enrollment.
This savings plan may NOT be combined with dental insurance benefits, any other discounts, promotions, or outside financing plans (Chase Health Advantage, Springstone Financial, CareCredit, and Dentalbanc)

Benefits are limited to treatment provided by PDA.
